A Note from Nurse Janis:
Remember to turn in medications,
* with pharmacy label (prescription)
*brand new unopened package(over the counter)
to your school nurse BY Jan 16. To expedite this process, have your medication authorization form already filled out and signed when turning it in to nurse.
Depending on your child, you may consider sending medications for: stomach, pain, motion sickness, diarrhea, allergies, or any other condition your child may experience.
Also, remember that if your child uses a topical medication (lotions, creams for skin conditions), this also needs medication authorization. Ask MD to authorize/check the "carry on person" and "self administer" boxes so that child can just use it as needed.
If your child wears contact lenses, send an extra pair or two. contact solution is NOT a medication and does not need med authorization, thankfully!

TURN MEDS IN EARLY TO AVOID PROBLEMS!!! Since we leave late on Jan 18, I cannot accept any medications late. Direct any questions to your school nurse. They will be able to contact me if there are any questions that they cannot answer
